The Middle School offers the following interscholastic sports teams for the 2011-2012 School Year.

Cross Country; Boys 7and 8 Combined Boys Basketball;7 and 8 combined
Cross Country; Girls 7 and 8 combined Girls Basketball; 7 and 8 combined
Volleyball; 7 and 8 combined

Student-Athletes must have the following:
1. The Middle School Athletic Fee has been set by the Board of Education for the school year 2011-2012 in the amount of $To be announced. This athletic fee is non-refundable.
a. You may pay by check to Nordonia Middle School Athletics or EZPay online, however please do not use the EZPay online option until after August 12th.
b. Athletic fees must be paid before a uniform is issued.
c. If your family is on free/reduced lunch program a Fee Waiver Form is available in the athletic office. This form must be filled out to have the athletic fee waived. The Fee or Waiver Form must be submitted before a uniform is issued. Kbr>2. A completed physical examination form (within the past year of the sports season) must be on file in the athletic office before participation in practice or tryouts.
a. It is mandatory that an athlete have a physical and questionnaire completely filled in and signed by the first day of practice/tryout. Due to the expanded question-and-answer section on family history, student- athletes are to complete the front page of the form along with their parents or guardians prior to their physical exam. Also, on the back of the form, please make sure that the examiner includes the date the exam was performed beside his or her signature. Lastly, you are reminded that there is a place on the form for parents and students to sign which states that you have read and understand the OHSAA Athletic Eligibility Information Bulletin.
3. A completed Emergency Authorization Form
4. A completed insurance waiver/risk of injury form
5. Academic Eligibility: a. A student in the 7th grade, for the first time, will be eligible for the first grading period regardless of previous academic achievement. Thereafter, in order to be eligible, a student in grade 7 or 8 must be passing at least 5 classes.
6. An athlete must reimburse the school district for the replacement cost of lost, stolen, or damaged equipment from previous sports seasons before he/she will be permitted to begin participation in any other athletic activity.

On Tuesday, August 23, 2011, there will be a mandatory meeting for the parents of any student interested
in participating in sports during the fall, winter, or spring sports� seasons.
The meeting will be held in the main gym at the middle school and will last from 5:00 PM to 5:30 PM. The
meeting will consist of a review of the student-eligibility bulletin, key essential eligibility requirements, and a review of the school�s Athletic Code of Conduct. The meeting will have a
sportsmanship, ethics, and integrity component as well as to give you the opportunity to meet the
coaches at the middle school. The meeting will also provide you with an overview of the upcoming year activities of the Nordonia Athletic Boosters.
There will be no sign-up procedure for fall sports. Student-Athletes wishing to participate, simply attend the first practice, dressed appropriately, with a completed Physical Form, EMA, Insurance Waiver, and Athletic Fee.
